The Holy Grail was the item used by Sailor Moon to transform into Super Sailor Moon. It had different origins in the manga and anime.
In the manga and Crystal, the Grail appeared when the hearts of Sailor Moon, Tuxedo Mask, and Sailor Chibi Moon came together. Its appearance broke the spell Cyprine and Ptilol had put on the other Sailor Senshi and, with everyone's hearts and power combined, allowed Sailor Moon to power up into Super Sailor Moon. At this time the talismans also resonated together in reaction to the power. In Act 36, Sailor Chibi Moon also received her own Grail that allowed her to transform into Super Sailor Chibi Moon during the final battle against the Death Busters. Without all of the Senshi present, Sailor Moon and Sailor Chibi Moon could not use the Grail to transform.
